We have begun to collect account and patient health information electronically. At your child’s appointment, we will ask you to complete forms for each scheduled child on a tablet with a secure connection to our digital charts, rather than paper as in the past. This may take a few extra minutes at your next appointment, but after that, verifying information should be quick and easy. We feel that this will provide us with the most accurate and up to date information to better serve your child and your family.
We have also added the option to complete this information online up to 2 weeks prior to your child’s appointment simply by pressing the Patient Login at the top of our website. Your login information will be emailed to you a couple weeks prior to your child’s appointment. This is a great option for you to complete the forms accurately ahead of time if you are unable to attend the visit with your child, or you would just like to save time when you arrive.
